    Implementation and evaluation of the VA DPP clinical demonstration: protocol for a multi-site non-randomized hybrid effectiveness-implementation type III trial.

    BackgroundThe Diabetes Prevention Program (DPP) study showed that lifestyle intervention resulted in a 58% reduction in incidence of type 2 diabetes among individuals with prediabetes. Additional large randomized controlled trials have confirmed these results, and long-term follow-up has shown sustained benefit 10-20 years after the interventions ended. Diabetes is a common and costly disease, especially among Veterans, and despite strong evidence supporting the feasibility of type 2 diabetes prevention, the DPP has not been widely implemented. The first aim of this study will evaluate implementation of the Veterans Affairs (VA) DPP in three VA medical centers. The second aim will assess weight and hemoglobin A1c (A1c) outcomes, and the third aim will determine the cost-effectiveness and budget impact of implementation of the VA DPP from a health system perspective.Methods/designThis partnered multi-site non-randomized systematic assignment study will use a highly pragmatic hybrid effectiveness-implementation type III mixed methods study design. The implementation and administration of the VA DPP will be funded by clinical operations while the evaluation of the VA DPP will be funded by research grants. Seven hundred twenty eligible Veterans will be systematically assigned to the VA DPP clinical demonstration or the usual care VA MOVE!® weight management program. A multi-phase formative evaluation of the VA DPP implementation will be conducted. A theoretical program change model will be used to guide the implementation process and assess applicability and feasibility of the DPP for VA. The Consolidated Framework for Implementation Research (CFIR) will be used to guide qualitative data collection, analysis, and interpretation of barriers and facilitators to implementation. The RE-AIM framework will be used to assess Reach, Effectiveness, Adoption, Implementation, and Maintenance of the VA DPP. Twelve-month weight and A1c change will be evaluated for the VA DPP compared to the VA MOVE!ProgramMediation analyses will be conducted to identify whether program design differences impact outcomes.DiscussionFindings from this pragmatic evaluation will be highly applicable to practitioners who are tasked with implementing the DPP in clinical settings. In addition, findings will determine the effectiveness and cost-effectiveness of the VA DPP in the Veteran population

    Evaluating diverse electronic consultation programs with a common framework.

    BackgroundElectronic consultation is an emerging mode of specialty care delivery that allows primary care providers and their patients to obtain specialist expertise without an in-person visit. While studies of individual programs have demonstrated benefits related to timely access to specialty care, electronic consultation programs have not achieved widespread use in the United States. The lack of common evaluation metrics across health systems and concerns related to the generalizability of existing evaluation efforts may be hampering further growth. We sought to identify gaps in knowledge related to the implementation of electronic consultation programs and develop a set of shared evaluation measures to promote further diffusion.MethodsUsing a case study approach, we apply the Reach, Effectiveness, Adoption, Implementation and Maintenance (RE-AIM) and the Quadruple Aim frameworks of evaluation to examine electronic consultation implementation across diverse delivery systems. Data are from 4 early adopter healthcare delivery systems (San Francisco Health Network, Mayo Clinic, Veterans Administration, Champlain Local Health Integration Network) that represent varied organizational structures, care for different patient populations, and have well-established multi-specialty electronic consultation programs. Data sources include published and unpublished quantitative data from each electronic consultation database and qualitative data from systems' end-users.ResultsOrganizational drivers of electronic consultation implementation were similar across the systems (challenges with timely and/or efficient access to specialty care), though unique system-level facilitators and barriers influenced reach, adoption and design. Effectiveness of implementation was consistent, with improved patient access to timely, perceived high-quality specialty expertise with few negative consequences, garnering high satisfaction among end-users. Data about patient-specific clinical outcomes are lacking, as are policies that provide guidance on the legal implications of electronic consultation and ideal remuneration strategies.ConclusionA core set of effectiveness and implementation metrics rooted in the Quadruple Aim may promote data-driven improvements and further diffusion of successful electronic consultation programs

    Anticipatory action on the various changes that occur is an action that cannot be bargained to be done immediately. The changes are larger, more diverse, and almost certainly, and are ongoing, including changes in the organization's management system, especially human resource management. The success of an organization, both private and governmental organizations in achieving its goals is largely determined by anticipatory measures to counteract the adverse effects of the change. This research was conducted at Jakarta Population Department. Specifically, the object of research took place in five tribal areas of the Department of Population, namely: Population Office of West Jakarta Municipality, Central Jakarta Population Subdistrict, East Jakarta Population SubDepartment, South Jakarta Population Duty Sub-Office, and Sub-Province Population of North Kotamadya .The results of this study are: The effectiveness aspects of the performance evaluation that are declared effective is the relevance factor. While that is not effective is the sensitivity, reliability, acceptability, and practicality. Very ineffective is the feedback aspect to the results of the performance evaluation of employees. The existence of the same perception among assessors, and priceless relating to the condition of effectiveness of performance evaluation implementation. This means that the assessor, and the valuable agree that there is a suitability of the aspects of effectiveness in the implementation of its performance evaluation in the DKI Jakarta Population Agency.     Cost Effectiveness Evaluation and Public Sector Treasury Single Account Policy Implementation in Nigeria

    Government policies and projects in Nigeria have most times been implemented at a very high cost. Sometimes the policy or project is still below average performance or at the preliminary stage, yet the total budgeted cost has been expended.  In this way most projects and policies are abandoned half way or variations of about half the initial contract sum is applied for to complete the project or give effective implementation to the policy.  Though, the enormous benefits of TSA policy as it is being expected in Nigeria, there is need to evaluate the cost effectiveness of its implementation.  The paper thus examined, cost effectiveness evaluation and  Treasury Single Account Policy implementation in Nigeria.  It x-rayed the meaning of TSA, the need for cost effectiveness evaluation of TSA policy, steps in cost effectiveness evaluation, TSA policy and the cost of implementation in Nigeria.  The paper concludes that applying cost effectiveness evaluation in TSA policy implementation enhances the net financial position of government and also it enhances the monitoring of the funding system of TSA policy as well as the performance level of its implementation.  We therefore recommend that cost effectiveness evaluation should be regularly carried on the TSA policy implementation and other government policies/projects and ensure actions are taken based on recommendations as this will boost public confidence on government, also government should ensure that cost effectiveness evaluation is carried out at the various stages of the TSA implementation as this will enhance the net financial position of government and finally, government should utilize cost effectiveness evaluation to monitor the funding of TSA policy implementation and assess its performance level at all times.     The purpose of this paper is to describe the development of student worksheet in English based on constructivism using problem solving approach for mathematics learning on the topics of social arithmetic, and its quality. The development of the student worksheet follows ADDIE model, there are: analyze, design, development, implementation, and evaluation. The instruments of the research are validation questionnaire, student response questionnaire, learning observation sheet, teacher interview, and test. The development process of student worksheet is: 1) analyze: need analyze, student analyze, and task analyze, 2) design: determining the component of the student worksheet based on analyze phase, 3) development: writing the student worksheet, doing expert validation, and revising student worksheet based on the validation, 4) implementation: implementing the learning process using student worksheet to examine its effectiveness and practicality, 5) evaluation: analyzing its effectiveness and practicality, and revising the student worksheet. The quality of student worksheet is: 1) the level of validity is 4,01, of 5 scales (valid), 2) the level of effectiveness is 80,56% (very effective based on student test), 3) the level of practicality are 81,6% (practice based on learning process observation), and 3,03 of 4 scales (practice based on student response).     The purpose of this study is done to determine the level of work units, central governmentsupport, guidance SAKIP evaluation of implementation, and quality and quantity of humanresources of the effectiveness of the implementation of SAKIP. This research is quantitativeresearch, collected by survey through questionnaires. The sample consisted of employees amountedby 110 respondents. This study sampling technique using non-probability sampling with saturated samples (sampling census).     The role of Framework Programmes in commercialisation and innovation growth shown through research results – evaluation research of the Polish effects

    The article presents the results of evaluation research whose aim was the assessment of the effects of completed projects implemented by Polish research departments within the European Union Framework Programmes. This evaluation is the first attempt at a comprehensive analysis of the effects of Polish projects backed by this type of financial support. The research was conducted on the basis of three evaluation structuring criteria: effectiveness, utility and sustainability. The analysis considers three consecutive Framework Programmes which enable the assessment of participation effects and evolution of experiences of Polish beneficiaries over time. The results of the research was juxtaposed with the results of similar evaluation research in other countries, which increased the objectivity of the conclusions. The research pointed to the high effectiveness of achieving strategic goals at institutional level and the career development of individual scientists, as well as low effectiveness in economic output, meaning the impact on innovation is low.     Impact of the Sierra Health Foundation's Clinic Capacity Building Program: Final Evaluation Report

    Sierra Health Foundation (Sierra Health) launched the Clinic Capacity Building Program in 2013 as part of the Sacramento Region Health Care Partnership. The goal of the Clinic Capacity Building Program was to respond to the anticipated growth in demand (i.e., number of patients) created by the implementation of the Affordable Care Act by strengthening community health centers' administrative and operational capacity. The program aimed to improve clinic leadership, care quality and financial sustainability, thereby increasing the number of high performing Federally Qualified Health Centers in the region.In July 2014, Sierra Health contracted with the Center for Community Health and Evaluation (CCHE) to evaluate the effectiveness of its Clinic Capacity Building Program. The goal of the evaluation was to assess the effectiveness of the Clinic Capacity Building program and contribution of the program to changes in capacity among the five grantees. This is the Executive Summary of the final evaluation report, which was submitted to Sierra Health in December 2015
